Requirements and Instructions for Public School Withdrawal Documentation 

Florida Statute requires the Florida Department of Education (FLDOE) to confirm that scholarship students are not simultaneously enrolled as a full-time public school student while receiving scholarship funding. 

Please continue reading for an overview of what documentation is acceptable and how to submit it.

✅ What documentation is acceptable?

At this time, the FLDOE mandates that ONLY the following documents will be accepted.

Only one document is needed per student. Documentation is only needed for students identified in the email. If you have a student who is not listed, no documentation is needed for them, and we kindly ask that you not submit it.

Students previously enrolled in public school:

• A completed Standard Withdrawal Form for Students Entering K-12 Scholarship Program. The form must be fully completed and include all required sections: the Parent Request for Withdrawal, the Public School District or Charter School Responsibility section (including the withdrawal date), and the school district or charter school administrator's signature. Students NOT previously enrolled in public school:
(Please provide ONE of the three required documents.)

• A letter from the student’s school district or assigned public school. The letter must include that the student has never been enrolled and a signature.

• An email from the student’s school district or assigned public school. The email must include that the student has never been enrolled and the email domain must match the school district (i.e., @hcps.net).
• A completed Standard Withdrawal Form for Students Entering K-12 Scholarship Program. The form must include the student information section and a school district or charter school administrator signature. Click here to view the form.

📝 What steps do I take to submit this documentation?

STEP 1: Contact your school district, assigned public school, or selected charter school to request the appropriate document listed above.

STEP 2: Upload the document to your Student Profile in EMA. Please be sure to select "Standard Withdrawal Form (2026-2027)" from the dropdown.

Reminder: Students without approved proof of withdrawal documentation by the deadline will not be presented for Quarter 1 and/or Quarter 2 funding and will not receive funding for those quarters.
