Deadline Approaching: Documentation needed by

11:59 p.m. ET on Tuesday, September 22, 2026

Dear Parent/Guardian,

Beginning in the 2026-27 school year, new students in first grade and above who are in a Scholarship Choice Confirmed status and have not yet been presented for Quarter 1 funding must upload an approved document in EMA confirming withdrawal from public school.

You are receiving this email because the document(s) submitted for your student(s), listed below, did not meet the requirements, as it was either incomplete or did not contain the required information needed to verify public school withdrawal.

To be presented for funding consideration for Quarter 1 and/or Quarter 2, you have one more chance to resubmit one of the approved documents confirming your student's withdrawal from public school by Tuesday, September 22, 2026, at 11:59 p.m. ET.

Additional documentation will not be accepted after the deadline, and incomplete or incorrect submissions will not be processed. Students without approved proof of withdrawal documentation by the deadline will not be presented for Quarter 1 and/or Quarter 2 funding and will not receive funding for those quarters.

Student Name(s): Name(s)

Please review the requirements and submit an acceptable document as outlined below.

Acceptable Withdrawal Documentation

Review the scenarios below to determine which document should be provided for your student(s) and the information required for each.

Students previously enrolled in public school:

  • A completed Standard Withdrawal Form for Students Entering K-12 Scholarship Program. The form must be fully completed and include all required sections: the Parent Request for Withdrawal, the Public School District or Charter School Responsibility section (including the withdrawal date), and the school district or charter school administrator's signature. Click here to view the form.

Students NOT previously enrolled in public school:
(Please provide ONE of the three required documents.)

  • A letter from the student’s school district or assigned public school. The letter must include that the student has never been enrolled and a signature.
  • An email from the student’s school district or assigned public school. The email must include that the student has never been enrolled and the email domain must match the school district (i.e., @hcps.net).
  • A completed Standard Withdrawal Form for Students Entering K-12 Scholarship Program. The form must include the student information section and a school district or charter school administrator signature. Click here to view the form.

For more information about the required documentation and upload instructions, please click here or watch a video here.

How to Resubmit Documentation

When you are ready to resubmit, you will need to delete the previously submitted document from EMA before you are able to upload the new one.

To do this, you will simply click the "Delete" button next to your previously submitted "Standard Withdrawal Form (2026-27)" document on your Student Profile in EMA. Once deleted, you will be able to select "Standard Withdrawal Form" again from the dropdown in EMA.

Note: Students who have accepted their scholarship and who are found by the FLDOE to be enrolled full-time in a Florida public school will forfeit their scholarship for the entire year.

Best,
